Grey's Anatomy expands into West Texas with new rural hospital spinoff

Everyone's favourite medical dramatic event, Grey's general anatomy is getting a spinoff — and this clip, it'll be go down in the dust-covered, rugged terrain of cowboy country.

The show's broadcaster, ABC, announced it has greenlit the currently untitled series, which is expected to premiere during the 2026-2027 broadcast season. ABC says it gave the one-hour drama a "straight to series order" — meaning the network is skipping the traditional standalone pilot episode and moving directly to a full season.

The official logline for the new series promises an "edgy drama about a team at a West Texas rural medical center — the last chance for care before miles of nowhere." 

That particular choice of setting has sparked mixed reactions from fans, many of whom are already speculating about how the spinoff could connect back to the original series. Several former main characters from Grey's Anatomy ended up in places like Boston, Switzerland and Paris, making Texas feel like an unexpected stop in the franchise's universe for some viewers. 

One person on X questioned the decision, writing, "Who is even in Texas?"

Others, however, welcomed the change of scenery from the original show's Seattle backdrop, with one X user sharing, "Shonda bringing the drama to the desert with rural hospital chaos is actually genius. Last chance medical center before miles of nothing? The hallway walks are about to hit different with tumbleweeds and cowboy boots."

While the setting might be changing, the new series appears determined to keep the same creative magic behind the scenes.

The spinoff will keep Grey's Anatomy mastermind, Shonda Rhimes on board as co-creator and executive producer, alongside Meg Marinis. The original show's longest-running star, Ellen Pompeo, will also executive produce the spinoff, along with longtime Rhimes collaborator Betsy Beers. 

So far, there's been no official word on how closely the new series will tie into Grey's Anatomy — or whether Pompeo or other familiar faces will appear on screen.

Marinis said she is "incredibly excited to expand the Grey's Anatomy' universe." 

"This opportunity will bring new characters and stories to life that will embody the same heart, emotion and connection audiences have loved from ‘Grey's' for more than two decades — all set in my home state of Texas," she said in a release.

The upcoming series, produced by Shondaland and 20th Television, marks the fourth spinoff of Grey's Anatomy, which just wrapped up its 22nd season. The longest-running running primetime medical drama in TV history also spawned Private Practice (2007-2013), Station 19 (2018-2024) and Grey's Anatomy: B-Team (2018).
